I have been a nurse for 16 years with the majority of my nursing in an inpatient hospital setting. I have worked in areas such as ER, cardiac,bariatric post op, outpatient endoscopy,  dementia.elderly, inpatient skilled nursing and the majority of my career in med/surg and  orthopedics. I have also worked float pool and agency so I have a variety of experience. Most of my years were spent on med/surg. I have much experience with cardiac Cath patients, post op including plastic surgery, general surgery, ENT, orthopedics; joint and spine. A large focus on chronic conditions such as COPD, Cirrhosis, CHF, diabetes. I have done over 6 years in various home care positions, especially skilled nursing with a lot of experience with post CABG patients, wound care, drain care, IV antibiotics, ostomies, wound vacs, medication reconciliation and set ups. I have always wanted a more focused approach to home care. Currently I still work in the outpatient setting in a discharge clinic and post intensive care syndrome clinic helping make the transition from hospital to home easier. However this is not enough. It has made me realize how many patients are unprepared to go home on their own and recover. The discharge process is rushed, patients are left feeling overwhelmed and worn out, some feeling as if though discharge was so soon. This is where I want to Bridge your gap.
