If you're discharging from the hospital and overwhelmed with instructions or unsure how to navigate the process I can meet you at the hospital to listen in, ask questions, take notes so you are ready to go home and can fully understand your care once you arrive home. Wound care , dressing changes, and assessment. Instructions on dressing changes and education on signs and symptoms of infection. Education on pain control and any restrictions. Personal assistance and care offered. Drain care and drain removal with your doctors orders.
Wound care, dressing changes and patient education. Can assist with general wounds, chronic wounds, wound vacs, ostomies, tube feeding and more. What signs and symptoms of infection you should monitor for. Any restrictions you at have.
Includes full assessment of medications, weekly or biweekly medication set up in your planner. Education and side effects of your medications. Can assist with continuous glucometer placement and set up. Having trouble with your Subq or IM injection medications. This is a separate service but can be included in your care plan if you are receiving other services This includes the nurse administering the injection or showing you how to safely do it yourself if you want to learn.
For conditions such as CHF, COPD, diabetes, Cirrhosis or liver disease, kidney disease or any other health conditions that require additional monitoring or education. Education provided as well as signs and symptoms to watch for and prevention of exacerbations
Injections Administration and teaching of Intramuscular and subcutaneous injections. If you're having trouble giving yourself injections these can be set up or teaching available to get you comfortable doing these yourself. Requires your physicians written order. Fertility injections set up according to your schedule.
Stim PIO and trigger shots. These require your drs orders and medication to be supplied. Times are flexible for your needs extra fees apply for times outside outside office hours. Packages and discounts available for multiple injections
Education on insulin and GLP-1 injections, site rotations. Education and review of the medications.
Administration of your blood thinner such as lovenox injections require your drs orders and supplies. Teaching provided as well.
Other Services
I do not offer caregiver services only nursing, however personal Care is important in recovery. If you need assistance with dressing, bathing getting a few extra things done around the house this service can be added on during your visit. Planning meals, knowing and understanding what you should eat according to the diet your doctor wishes you to be on I can help you plan this out.
Need DME's, services offered through home health or Medicaid, we can look at this with additional resources to see what might be offered to you through your insurance. Transportation to and from appointments or elsewhere we can look at these options. Transportation also may be offered with your care plan. If you need additional private duty services such as extended nursing visit, caregivers. I can refer you to some companies or help you look to see what best fits you. These visits can be offere
If your doctor has ordered labs for you I can assist you to the lab to have this done as well as making the appointment. If you need to have a urine or stool or other specimen dropped off I can pick up your cup and deliver it to the lab for you. Need medications picked up, if there are no pharmacy restrictions I can assist with picking up and bringing you your medications.
